Major Function :
The Licensed Vocational Nurse (LVN) provides direct nursing care to clients as prescribed by the physician, under the direction and supervision of the RN. Nursing activities are limited to those in compliance with the State LPN/LVN Practice Act, any applicable licensure/ certification requirements and Lorian Home Systems, Inc. of Las Vegas (LORIAN) policies and procedures.
Qualifications:
- Graduate of an approved school of practical/vocational nursing.
- Certified/licensed and in good standing within state currently working in according to clinical type.
- One (1) year experience as a professional nurse.
- Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
- Has the ability to read and follow written instructions and document care given.
- Is self-directing and has the ability to work with little direct supervision.
- Possesses and maintains current CPR certification.
- Is flexible and cooperative in fulfilling the role obligations.
- Demonstrates tact, patience, and good personal hygiene.
- Is a licensed driver and has an automobile that is insured in accordance with LORIAN requirements.
- Must be able to verbally communicate in the language of the client and document all client related activity practices.
Responsibilities:
- Provides those services requiring nursing skills in accordance with the plan of treatment or plan of care initiated by the RN.
- Notify the patient’s attending physician, dentist, podiatrist, or other professional persons and responsible staff of significant change in the patient’s condition.
- Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients condition and care given in accordance with LORIAN policies and procedures.
- Note: Documentation must be accurate, complete, and submitted to LORIAN before visit is deemed acceptable and payable to nurse.
- Educate and instruct the patient, patient’s family, or staff, as required, as outlined in and consistent with the LVN Practice Act.
- Ensure quality and safe delivery of home health services to patients/clients and families/ caregivers.
- Participates in coordination of home health services, appropriately reporting the need for disciplines (CHHA, OT, PT, ST, MSW, LVN, and/or Dietician) to the case manager and/or nursing supervisor.
- Participates in Case Conferences.
