Job Summary:
The Director of Quality Assurance oversees the overall functioning of Lorian Home Systems, Inc. of Las Vegas (LORIAN)’s Quality Assurance and Medical Records Departments ensuring that documentation of the actual care provided to patients is timely, complete, and accurate. Makes sure documentation is completed in accordance with Federal and State guidelines and LORIAN’S own policies and procedures and that the medical chart of every patient admitted to LORIAN is accurate, complete, and up to date.
Qualifications:
- Two to three years of actual experience in healthcare/homecare, preferred.
- Two years in a supervisory position, preferred.
- Knowledge of generally accepted homecare nursing principles and practices.
- Knowledge of Medicare and applicable state rules and regulations relative to home health services, including their coverage and delivery.
- Knowledge of Medicare and applicable state rules and regulations relative to the documentation of services rendered to patients in a home health setting, including content and timeliness requirements of the documentation.
- Specific knowledge of OASIS forms completion, including timeliness requirements, and ICD9 and V diagnosis coding.
- Ability to develop and formalize a plan of care (POC) for each patient admitted to LORIAN based on the data collected during the admission process through the OASIS form, medication Profile, and other forms included in the intake package.
- Knowledgeable of Medicare and applicable state rules and regulations for the development of execution of POC’s for patients receiving IV, wound, and catheter care services.
- Ability to lead, to supervise, to guide, and to develop the skills and performance of the Quality Assurance and Medical Records department staff.
- Ability to exercise independent judgment relative to home health compliance and eligibility issues.
- Ability to evaluate staff after successful completion of the 90 day introductory period and at a minimum once a year thereafter.
- Ability to design and implement the required infrastructure and policies and procedures to ensure LORIAN’S on-going compliance with prescribed Medicare and applicable state rules and regulations in terms of documentation and patient care.
- Ability to provide training and education for LORIAN’S clinical and office support staff relative to Medicare and applicable state regulation compliance matters.
- Ability to interact with other LORIAN departments to ensure an effective, efficient, and regulatory compliant delivery of services to patients.
Responsibilities:
- Manages and oversees the overall functioning of the Quality Assurance and Medical Records Departments.
- Ensures that intake documentation, including OASIS, Physician Orders, etc. are prepared and turned in by the clinical staff on a complete, accurate, and timely manner and that the same documentation is processed by the Quality Assurance and Medical Records Departments in a similar fashion.
- Collaborates with the Director of Nursing and clinical supervisors to ensure that all OASIS assessment forms, including discharges, transfers, resumptions, etc. are promptly and accurately prepared by the field staff and processed in a similar manner by the Quality Assurance and Medical Records Departments.
- Ensures that a POC is prepared in a timely and proficient manner for every patient admitted to LORIAN based on the documentation and information gathered by the clinical staff during the intake, re-certification, or resumption of care process.
- Ensures that orders on the POC are specific enough to meet compliance requirements of Medicare and applicable state rules and regulations, specifically as they relate to IV care, wound care, and catheter care patients.
- Ensures the proper and accurate diagnosis coding of all patients admitted to LORIAN and collaborates with LORIAN’S Director of Nursing, Clinical Supervisors, and Intake staff to ensure that all patients are properly staffed and that proposed visit frequencies are prudent and reasonable given the patient’s actual diagnosis, condition, and acuity.
- Ensures that medical records for each and all patients are accurate, complete, and maintained up to date.
- Ensures that Medical Records staff is constantly in contact with field staff to ensure that OASIS forms, follow up visit notes, physician orders, communication notes, etc. are prepared and promptly turned in to, and processed by, the Medical Records department.
- Ensures that follow up visit notes are turned in by the responsible clinical staff in a timely fashion and that the notes are consistent and in agreement with the corresponding plans of care.
- Ensures that Lorian’s Director of Nursing, Clinical Supervisors, and Intake staff are informed of instances of non-compliance on the part of the clinicians in relation to the timeliness, accuracy, and completeness of clinical documentation submitted.
- Ensures that Quality Assurance and Medical Records staff is periodically evaluated for performance in accordance with Lorian’s policies and procedures.
- Performs such other duties as assigned.
