Job Summary:
The Intake/Scheduling Coordinator position supports the Clinical Management Team in all department functions with a focus on accepting and processing all referrals, Durable Medical Equipment (DME) requests from field staff, and lab results. This position is also responsible for the scheduling department with effective coordination of appropriate field staff for servicingpatients.
Qualifications:
- Currently licensed to practice as a LVN in the state of CA.
- 1-2 years of clinical experience in a home care setting preferred.
- Must have professional and customer-service-driven phone skills.
- Communicates effectively with clinical and office staff in a constructive, goal directed, and professional manner.
- Ability to present reports to small and large groups of customers upon request.
- Ability to be flexible, to follow verbal and written instruction, and to work in a team oriented environment.
- Strong knowledge of Word, Excel, and computerized systems in general required.
- Ability to develop and maintain filing systems.
- Participates in maintenance of quality, safety, and/or infection control standards.
- Proficient in completing routine paperwork, multi-tasking, and providing appropriate follow-up as needed.
- Maintain confidentiality of records and information at all times.
Responsibilities:
- Ensures all relevant patient information is obtained in an accurate and timely fashion when new patients are referred to Lorian Home Systems, Inc. (LORIAN) by the referring provider or marketer.
- Ensures all relevant patient information is entered into LORIAN’S patient tracking system (VisiTrack) when received from referring provider or marketer.
- Provides supportive documentation to LORIAN’S Billing Department when determining insurance eligibility.
- Works with the billing department to determine patient eligibility for services.
- Works closely with LORIAN’S Billing Department to ensure that LORIAN services do not overlap another agency’s services.
- Regularly collaborate with the Clinical Management Team in reviewing new referrals and determining the appropriateness of any given referral for the agency.
- Ensures all initial patient profiles in VisiTrack using unique identification numbers according to established Medical Records protocol.
- Coordinates the assembly of patient admission packets to include patient profiles and any and all supporting documentation obtained from referring provider and/or marketer.
- Provides all relevant patient information (i.e. patient personal demographic, history and physical, current medications, physician’s orders, surgical reports, etc.) obtained upon referral to the Medical Records Department for the creation of the patient’s chart.
- Assigns staff to newly admitted patients and regularly participates in the maintenance and accuracy of the scheduling board where all new admissions are concerned.
- Collects all staff schedules weekly and inputs into VisiTrack for appropriate staff scheduling and assignment based on patient needs and demographics.
- Ensures computerized schedules for patients are kept up to date in VisiTrack on an ongoing basis.
- Maintains accurate written records in VisiTrack on all patients not admitted and may be required to provide that information to a Clinical Manager upon request.
- Process all DME requests from clinical field staff to include contacting vendors, faxing orders, and following up with the ordering clinicians once complete.
- Serve as a liaison between the DME vendors and field staff.
- Receives all processed lab results from community/ hospital laboratories (via mail / fax), fax to ordering physicians, place copies in case managers LORIAN mailboxes, ensure a copy is placed in the patient’s medical record, and call physicians with STAT results obtained.
- Troubleshoot problems with lab specimens, requisitions, and any other issues that arise in regards to labs from both our internal and external customers.
- Provide appropriate feedback to referring provider and marketers related to non-admits and/or refusals.
- Appropriately makes arrangements for services required by patients that are not offered by LORIAN.
- Ensures compliance with all State and Federal regulations/ practices.
- Attend and participate in all agency wide meetings/ in-services related to the Intake/Scheduling coordinator position.
- May perform other duties as assigned by management staff.
