CLINICAL MANAGER

Job Summary:

 The Clinical Manager is a Registered Nurse (RN) who has graduated from an accredited school of nursing and is currently licensed to practice in the state of Nevada; a practicing physician; or a health care professional with equivalent experience. He or she assists the Director of Nursing in supervising home health services to homebound patients in their place of residence in accordance with attending physician orders and plans of care and strives to provide the highest quality of care. If an RN, the Clinical Supervisor is available at all time during operating hours and participates in all activities relevant to professional services furnished, including the development of qualification and assignment of personnel.

Qualifications:

  • Registered by the state(s) where currently practicing as a RN, if a RN.
  • Three to five years of experience in health care/homecare, preferred.
  • Two years in a supervisory position, preferred.
  • Knowledge and ability to apply community health principles and practices.
  • Knowledge of Lorian Home Systems, Inc. of Las Vegas (LORIAN) policies and procedures.
  • Ability to supervise, guide, and develop skills and performance of service personnel.
  • Ability to exercise independent judgment.
  • Ability to work with individuals.
  • Ability to assist in evaluating personnel a minimum of one time per year.
  • Ability to deal effectively with high levels of stress.

Responsibilities:

  • Ensures that patients are being admitted to LORIAN in accordance with LORIAN’S policies and procedures and that the needs of all new patients are being properly attended to based on the patient’s initial assessment.
  • Review patient pending list on a daily basis.
  • Ensures that a Plan of Care (POC) is promptly developed for each new patient admitted to LORIAN and that the POC properly addresses the patient’s needs as determined by the patient’s initial assessment.
  • Ensures that each patient’s POC is being properly executed and that the continuity and effectiveness of care for each patient is maintained by reviewing the patient’s chart and communicating with the clinical staff caring for the patient on an on-going basis.
  • Responsible for ensuring that patients are being discharged from service in accordance with the POC and that both patient and physician are being informed of discharge plans and patient’s actual progress toward care objective identified on patient’s POC.
  • Ensure that actual care being provided to patients meets State and Federal guidelines by performing supervisory visits as needed in cases where licensed vocational nurses and/or home health aides are the primary giver and is available to perform patient visits as deemed necessary by the DON.
  • Work closely with the intake department to ensure that patients referred to LORIAN are being promptly admitted and, once admitted, visited in accordance with prescribed frequency.
  • Responsible for ensuring that follow up visiting notes, communication notes, and prescribed doctor’s orders are turned in by clinical staff in a timely and complete manner and in accordance with LORIAN’S policies and procedures.
  • Work closely with the quality assurance department to ensure that care provided to patients meets prescribed State and Federal guidelines and is being documented on the patient’s chart fully and timely.
  • Reviews and ensures accuracy of coding for the patient’s chart fully and timely.
  • Reviews and ensures accuracy of coding for Medicare patients.
  • Ensures that all patients’ claims are ready for final billing as soon as patients are discharged from service by LORIAN.
  • Work closely with the billing department to ensure that the billing function is expedited and optimized.
  • Work with the intake department to ensure that Treatment Authorizations are obtained for Medi-Cal and other insurance patients upon admission.
  • Observes confidentiality and safeguards all patient related information.
  • Accepts responsibility for regular attendance, punctuality, and fulfills job requirements without regard to time involved.
  • Serves as a resources person to employees.
  • Attends pertinent continuing education programs other than routine in-services and shares information with staff.
  • Performs such other duties as assigned.
